About This Opportunity
The Cardiovascular Health Awareness Program (CHAP) was designed to support prevention of cardiovascular disease (CVD) and stroke in communities. Older adults are invited to attend sessions in local pharmacies, churches and community centers for a cardiovascular risk assessment including blood pressure monitoring.
Responsibilities include, setting up blood pressure machine, maintaining client confidentiality, practice infection control procedure, filling out agency forms as necessary and communicate with Wellness team and supervisor. Additionally, attend any ongoing trainings report any concerns to the program assistant or supervisor,
First Thursday of each month from 11am-12pm
