Scope & Needs
- Advanced skills/familiarity with the online form builder JotForm.
- The scope of work includes the development of three separate forms (e.g., adoption application form) that require integration with an e-signature tool and advanced conditional logic.
- Form content has been drafted and approved and an AVA lead is assigned to this project to answer questions.
- Ongoing tech support is not required beyond the creation of the three forms.
