TIMEFRAME: Long-term commitment required. Times of programs vary throughout the week. Programs run for 6-week blocks throughout the year.
LOCATION: Varies. Programs run both in-person and virtually. Some travel is required in the Erie St. Clair region (Windsor-Essex, Chatham-Kent, Sarnia-Lambton).
POSITION SUMMARY: Peer leaders are community members who have lived experience with a chronic health condition or care for a person with a chronic health condition. Peer leaders will complete training in order to lead and facilitate workshops for others experiencing similar chronic health conditions. Peer leaders will receive the following:
- A stipend for every series led and completed, for those who qualify
- All training materials required to become certified
- All materials and resources to facilitate the workshops
- Completion of a professional training to lead the registered workshop, as it is offered to people in the same away around the world. (ie workshops are licensed materials and protected with strong fidelity rules)
- Co-lead one of the following workshops with another peer leader trained in the same program

- Master Your Health (Chronic Conditions, Chronic Pain, Diabetes)
- Work with a variety of people i.e: different educational levels, cultures, and physical or mental challenges
- Respect and adhere to personal privacy law and maintenance of nondisclosure of workshop participation personal information
- Act as a positive role model for the workshop participants and are be willing to consistently apply self care management actions and principles in their own lives
- Perform other duties that support the mission and vision of the organization and/or as assigned
based on position responsibilities and/or site requirements

- Able to problem solve
- Able to maintain confidentiality
- Committed to the philosophy of the program, its objectives and goals
- Able to facilitate group discussion and communicate about sensitive issues
- Be engaged in the community in some capacity already as an indication of a desire to be activated as a benefactor of community work.
- Bilingual (English/French) language skills an asset.
- Healthcare or education background is beneficial, but not required
- 18 years of age and over
- Long-term commitment required
- Have lived experience with a chronic health condition or care for a person who cares for a chronic health conditions
- Have participated in any one of the self-management workshops:
- Powerful Tools for Caregivers
- Diabetes, Healthy Feet & You
- Master Your Health (Chronic Conditions, Chronic Pain, Diabetes)
- Completion of a professional training to lead the workshop(s) (costs covered)
- Current and valid Class “G” Ontario Driver’s License with reliable transportation to support travel throughout ESC region.
- Completion of a volunteer application and screening interview
- Provide a Police Vulnerable Sector Check (valid within the last year)
- Complete a 2-step TB Skin Test
- Provide Covid-19 Vaccination receipt (minimum 2 doses)
- Complete weCHC orientation requirements, including Immunization and Screening Form

- Driver's License
- Reliable transportation to support travel throughout Erie St. Clair region
- Completed application
- Completed screening interview
- Vulnerable Sector Check (within the past 1 year)
- Complete a 2-Step TB Skin Test
- Proof of Covid-19 Vaccination (2 doses)
- Willing to complete medical forms
